Pulsatile Innovations Media Release
& Consent Form

Pulsatile Innovations collects and shares real stories, insights, and experiences from individuals, families, and professionals whose lives have been touched by cardiac arrest. These stories may be used in interviews, videos, written articles, photography, or other media formats to raise awareness, educate the public, and promote safety, innovation, and improved outcomes in emergency response. Content may also be used to support Pulse Tile™’s broader mission, including marketing, public relations, investor engagement, and fundraising activities, both now and in the future.

Consent

I, the undersigned, hereby grant Pulsatile Innovations Pty Ltd (Pulse Tile™) and its representatives the right to:

Record my image, voice, likeness, and/or written or spoken words during interviews, photography, audio, or video recordings.
Edit, adapt, reproduce, publish, and distribute these recordings in whole or in part, without restriction, for marketing, educational, or promotional purposes.
Use my name, story, and any information voluntarily shared in connection with the media produced.

I understand that:

The content may be used across multiple platforms, including but not limited to websites, social media, print materials, and media presentations.
The content may be shared with partners, media outlets, or third parties for promotional purposes aligned with Pulse Tile’s mission.
I will not receive payment or royalties for the use of this content.

Release:

I release Pulsatile Innovations Pty Ltd, its officers, employees, contractors, and assigns from any and all claims, demands, or causes of action that may arise from the use of the recordings or materials, including but not limited to claims for defamation, invasion of privacy, or infringement of rights.

Acknowledgement

I confirm that:

I am over 18 years of age (or my parent/guardian has signed below).
I have read and understood this release form.
My participation is voluntary, and I may withdraw consent before recording begins by notifying the interviewer.
Once published, I understand the content may remain publicly available indefinitely.
